Please note that the Site is directed towards users who reside in the United States. It is not our intent to gather personal information from individuals residing outside the United States. MINORS

Our Site is not intended for or designed to attract children under the age of 13. Moreover, we do not knowingly collect any personal information from anyone under the age of 18 without the consent of a parent or guardian, and you must be 18 years of age or older to submit registration or survey information. • For Providers: your name and contact information and medical specialty

In addition, when you request treatment from a Provider through the Site, you may share:

• Medical information submitted for diagnosis or treatment purposes (including information about your applicable medical history) and current complaint or other reason for visiting a Provider.

SOCIAL MEDIA PLUGINS

The Site may use social media plugins (e.g., the Facebook "Like" button, '"Share to Twitter"' button) to enable you to easily share information with others. When you visit the Site, the operator of the social plugin can place a cookie on your computer, enabling that operator to recognize individuals who have previously visited the Site. If you are logged into the social media website while browsing on our Site, the social plugins allow that social media website to receive information that you have visited our Site. APPLICABILITY OF HIPAA

As described above, you understand that Revive is not a "covered entity" as defined by HIPAA. The Pharmacies and Providers may be considered a "covered entity" or "business associate" under HIPAA, and Revive may, in some cases, be a "business associate" of a Pharmacy or Provider. If Revive is deemed a "business associate," we will be subject to additional protections of your "protected health information," as defined under HIPAA ("PHI") and outlined in our Business Associate Agreements with such Pharmacy or Provider. Your PHI will be used and disclosed only in accordance with such applicable laws, regulations, and contractual protections.

YOUR CHOICES

In General. We respect your right to make choices about the ways we collect, use, and disclose the information you provide on or through our Site. We strive to provide choices to you at a meaningful time and in a meaningful place. For example, if you don't want information about your visit to our Site sent to Google Analytics, you may download an Opt-out Browser Add-on. Please note that the Add-on does not prevent information from being sent to the Site.

HOW TO ACCESS, UPDATE, OR CORRECT YOUR INFORMATION

If you have created a profile on our Site, you may be able to access, update or correct your information through your account settings. Otherwise, you may need our help.
